Endotoxin (lipopolysaccharide, LPS) activates macrophages in vivo, leading to a lethal form of acute renal failure (ARF). Previous studies indicate that LPS administration leads to increased synthesis and release of endothelin (ET-1), which has diuretic and natriuretic effects in vitro. However, the effect of LPS on ET-1 and renal function in vivo is unknown. We found that renal medullary ET-1 content increased significantly during LPS-induced ARF, whereas the renal vein plasma ET-1 level did not change. Because the high renal medullary ET-1 concentration occurred simultaneously with the ARF, we hypothesized that ET-1 was the mediator of the lethal ARF.
